Your first appointment

Please arrive 15 minutes before your scheduled appointment to fill out any necessary forms and paperwork. Please bring any dental insurance information you may have with you. If you take antibiotics before dental work, be sure to take the medication 1 hour prior to your appointment. The initial visit will consist of a review of your medical history, a full comprehensive dental exam and taking any necessary radiographs. Dr. Kreitman and her staff are eager to answer any questions you may have. We look forward to meeting you and welcome you to our practice.

For your convenience, we offer the following forms that you can fill out ahead of time and bring in with you for your visit. Please feel free to download and print these forms before coming in. If you have any questions, feel free to give us a call.

Insurance & payment

We work with most major PPO dental insurance plans as an out-of-network provider. As a courtesy we will file the claims to your insurance provider on your behalf.

Please give us a call or email office@drkreitman.com with your insurance details before your first visit, and we'll verify your specific coverage and any estimated patient portion.

If you don't have dental insurance, we welcome you all the same — we'll be transparent about cost upfront and can coordinate third-party financing for larger treatment plans if you'd like to spread payments over time.
